Preferred Name Change Form
With the growing diversity of our campus, many students use names other than their given name for a variety of personal or cultural reasons. To better serve our students, we have created the option to add a Preferred First Name to:
- Class rosters
- Advising Guide
- Desire2Learn (D2L), KSU's learning management system and electronic classroom
By submitting the Preferred Name form through Owl Express, I understand that:
- The Preferred First Name replaces the Legal First Name in D2L learning management system.
- The Preferred First Name will be added to the Class Roster, Student Advising Guide and Degreeworks.
- Student's legal name will remain unchanged in all other university related systems.
How to Submit Your Preferred Name
All Preferred Name Change requests are to be submitted through Owl Express. The Preferred Name form is located under the Student Records tab.
The deadline to submit Preferred Name form is no later than 5pm on the First Day of Classes of the Fall and Spring semesters and the 8-Week session First Day of Classes of the Summer Semester. Refer to the Academic Calendar for specific dates.
Once submitted, the Office of the Registrar will process the request within 2-3 business days. The Office of the Registrar will confirm the receipt of the request and validate the request by emailing the student's KSU email account. Students are encouraged to check their KSU email account often for important messages from KSU.
Requests received after the deadline will be applied to the record the following semester.
Note: Preferred names will not be used in cases where legal name is required, including but not limited to, financial aid documents, official transcripts, diplomas, payroll records, and federal immigration documents.
